JO A. PEAY STUDENT INVOLVEMENT AWARD
CRITERIA
Eligibility for this award will be determined based on the following criteria and the completion of the attached questionnaire. Supporting documentation must be provided for verification.
1. The Sponsoring Chapter must provide a scholarship to their Student Chapter each year.
2. The Sponsoring Chapter must provide at least one program per semester/quarter.
3. Sponsor a new Student Chapter, whose members must be installed, with all dues and fees paid, between May 1, 2008 and April 30, 2009.
4. A member in the Sponsoring Chapter serve on the Office Technology Systems Advisory Board for the school where the Student Chapter is active.
5. Invite students to attend chapter meetings, APW events and NCD events. Sponsoring chapter may assist with fees associated with these meetings.

QUESTIONNAIRE
Please complete this questionnaire in detail, providing full documentation for verification. Documentation should include copies of meeting minutes, chapter checks to verify scholarship payment, chapter invitations to meetings/events., etc. Each question is worth 25 points. The Chapter with the largest cumulative score will win this award. In the event of a tie, the winner will be chosen by lot.
1. Did your chapter provide a scholarship to a Student Chapter member? Please include student name, the school they are attending and the amount of scholarship awarded.
2. Did your chapter present one or more program(s) per semester/quarter to the Student Chapter? Please include program name(s) and date(s) presented.
3. Did your chapter sponsor one or more NEW Student Chapter(s), and were the members installed with all dues and fees paid, between May 1, 2008 and April 30, 2009?
4. Did a member from the sponsoring chapter serve on an Office Technology Systems Advisory Board for the school where the Student Chapter is active?
5. Do you as the Sponsoring Chapter invite students to meetings, APW event(s), seminars? If so, do you waive the registration fee or have a special rate that you charge the student(s)?
